Medicare Part B Premium in 2027: What We Know
Part B is the one number that decides how much of your COLA you actually keep, because for most beneficiaries the premium is deducted directly from the Social Security check. This page tracks the premium: the confirmed 2026 figure, the 2027 projection and where it comes from, the recent history, and what happens the moment the official premium is published.
Standard Part B premium, recent years
| Year | Standard monthly premium | Status |
|---|---|---|
| 2022 | $170.10 | Official |
| 2023 | $164.90 | Official (a rare decrease) |
| 2024 | $174.70 | Official |
| 2025 | $185.00 | Official |
| 2026 | $202.90 | Official |
| 2027 | $209.50 | Projection (Medicare Trustees) — official figure expected November 2026 |
What happens when CMS announces
Three things follow the announcement, usually in November: (1) the official standard premium and the IRMAA income brackets are published together; (2) your December COLA notice applies the real premium to your January check; (3) this site replaces the $209.50 projection with the official figure everywhere the same day, and emails the update to subscribers. If the official premium comes in below the projection, net raises will be slightly larger than the estimates shown here — and if it comes in above, the hold harmless rule still protects most beneficiaries from a net decrease (explained with the check on the net COLA calculator).
IRMAA: the higher-income surcharge
About 8% of beneficiaries pay IRMAA — an income-related surcharge added to the standard premium. Your 2027 IRMAA tier is set by your modified adjusted gross income on your 2025 tax return (the two-year lookback). The 2027 brackets are published with the standard premium in November; until then, 2027 IRMAA amounts are projections and are not used in this site's calculators. Life-changing events — marriage, divorce, retirement, loss of income — can justify an IRMAA redetermination via Form SSA-44. The full bracket tables and the two-year lookback are on the IRMAA brackets 2027 page, and the protection that keeps a premium rise from cutting your net check is explained on the hold harmless rule page.

Frequently asked questions
What is the Medicare Part B premium for 2027?
It has not been announced yet. The official 2026 standard premium is $202.90 a month. The Medicare Trustees project $209.50 for 2027 — a projection, not the official figure. CMS is expected to announce the actual 2027 premium in November 2026.
When is the 2027 Part B premium announced?
CMS (Medicare) traditionally announces Part B premiums in November, after the COLA is known, because most premiums are deducted from Social Security checks. When the official figure is published, this site updates every affected page the same day.
How much of my COLA will the Part B premium take?
At the projected $209.50, the premium rise is $6.60 a month over the 2026 premium of $202.90. For the average retiree with a $75 gross raise, the take-home (net) raise is about $68. The hold harmless rule additionally guarantees that most beneficiaries whose premium is withheld from their check cannot see a net decrease.
What is IRMAA for 2027?
Higher-income beneficiaries pay an income-related monthly adjustment amount (IRMAA) on top of the standard premium. The 2027 IRMAA brackets are announced together with the standard premium and are based on your modified adjusted gross income from two years earlier — your 2025 tax return for 2027 premiums. Until CMS publishes them, any 2027 IRMAA figures are projections.
